Enrollment Forms

Welcome to ALP!

Enrollment is easy.  Please note that you will need the following information in order to complete registration:

  1. Household  information – physical address, phone numbers, proof of residency**
  2. Parent/Guardian information – birthdate, work and cell phone numbers, email addresses, photo ID
  3. Student information – birthdate, demographic and health information
  4. Emergency Contacts – names and phone numbers of trusted contacts
  5. Legal guardianship or custody papers, if applicable

**Proof of Residency – In accordance with Arizona Revised Statutes, school districts & charters are required to obtain and maintain verifiable documentation of Arizona residency for all students. Proof of residency documents need to match the same name as the person signing the registration form.  Acceptable Proof of Residency Documents include the following:

  • Valid Arizona driver’s license
  • Arizona identification card or motor vehicle registration
  • Real estate deed or mortgage documents Property tax bill
  • Residential lease or rental agreement
  • Water, electric, gas, cable, or phone bill
  • Bank or credit card statement
  • W-2 wage statement
  • Payroll stub
  • Certificate of tribal enrollment or other identification issued by a recognized Indian tribe that contains an Arizona address
  • Documentation from a state, tribal or federal government agency (Social Security Administration, Veteran’s Administration, Arizona Department of Economic Security)
